Instructions

Complete the sections as completely as possible, making sure to include the serial number for every club. Some clubs within the same set may have different serial numbers. Please ensure that all the clubs in the box are listed on this form. Please note that ALL COMPONENTS AND PARTS are required for no-charge consideration. If you need DIFFERENT repairs done to each club within a section, please describe this to us in detail in the Additional Instructions area. For example, please note which irons need reshafting vs. regripping.

Serial numbers for irons can be found on the hosel (where the shaft enters the clubhead), metal wood serial numbers on the sole or hosel, hybrid serial numbers on the sole or hosel, and putter serial numbers on the face, hosel or heel. If you cannot find your serial numbers, please contact the Repair Department for assistance.

If your club(s) have been altered by anyone other than PING you may incur additional cost or it may prohibit us from performing the requested repair.
